Adalimumab neutralizing antibody [Titer] in Serum by Bioassay
Active
Term Description
The result of this assay represents the minimum serum dilution at which adalimumab activity is no longer blocked by anti-adalimumab antibody in the patient specimen (i.e., the dilution at which the antibody's neutralizing effect is eliminated). The term is created for an anti-adalimumab antibody assay employing the cell-based iLite TNF-alpha detection kit, but the term is not restricted in use to that product.
Part Descriptions
LP173592-9 Adalimumab
Adalimumab is a human monoclonal antibody that binds tumor necrosis factor alpha (TNFα). TNFα is a cell signaling protein that, when present in excess, may lead to the development of inflammatory immune diseases including Crohn's disease, rheumatoid arthritis, psoriatic arthritis and inflammatory bowel disease. Adalimumab inactivates TNFα by binding to the TNFα receptor. PMID: 23943614 Treatment with adalimumab does not necessitate laboratory monitoring though association with increased transamines and pancytopenia may argue for intermittant blood cell counts and liver function tests. Adalimumab has been associated with serious infections including the reactivation of tuberculosis and deep fungal infections as well as other rare side effects, including lymphoma, heart failure and demyelinating disease. Neutralizing antibodies are antibodies that interfere with the interaction between the virus and its host cell receptor, therefore preventing infectivity. Neutralizing antibodies can be detected using a traditional VNT or other methods such as pseudovirus VNT and surrogate VNT. VNT and pseudovirus VNT use live virus (the actual virus or a recombinant virus that incorporates the viral antigen of interest, respectively) and cell culture to assess the virus-host interaction and whether neutralizing antibodies are present that block that interaction. Surrogate VNT (sVNT) does not use live virus but rather uses purified viral antigen and host cell receptor protein to mimic the same interaction. The sVNT method is currently (2021) represented in LOINC as IA, because it is an immunoassay-based method.

Antibodies to adalimumab were shown to develop in 67% of patients within the first 28 weeks of treatment. Only thirteen percent of patients with adalimumab antibodies experienced relief from symptoms compared to 48% of patients without antibodies to adalimumab. Only 4% of patients with antibodies went into remission compared to 34% of patients who achieved remission without anti-adalimumab antibodies. Antibodies to adalimumab reduced the likelihood that patients would have relief from disease symptoms or go into remission. PMID: 21486979
