A monoclonal band (M-spike, monoclonal protein isotype) is often found in patients with monoclonal gammopathies. Following protein electrophoresis, immunofixation is used to identify the immunoglobulin heavy chain and/or light chain. Monoclonal peaks greater than 3 g/dL and IgG or IgA are seen in patients with Multiple Myeloma (MM). A monoclonal peak greater than 3 g/dL and IgM is seen in patients with macroglobulinemia. An alternate methodology for identifying the monoclonal protein isotype is MALDI-TOF, in which known information regarding the unique mass of each isotype is used to determine and quantify the isotype present in the patient sample based on the mass spectra generated from the sample.

LP6118-6 Albumin
A non-glycosylated protein that is synthesized in the liver parenchymal cells, regulated by colloid osmotic pressure from interstitial fluid surrounding hepatocytes, and catabolized in nearly all organs. Albumin comprises >50% of plasma proteins in plasma, CSF and urine, and therefore is a key contributor to intravascular oncotic pressure and regulating fluid concentration in tissues. Albumin also binds and transports a variety of endogenous substances and medications. Serum albumin measurement is a reflection of hydration and nutritional status, blood oncotic pressure, liver and kidney function, and other chronic diseases, and the ratio of albumin in the CSF and serum can be used to evaluate blood brain barrier disorders. [Tietz] Urine albumin of less than 30 mg/day is considered normal. Microalbuminuria is defined as urine albumin of 30-300 mg/day and is an early sign of kidney damage from disorders such as diabetes or hypertension. Gamma globulins, or Ig's, are a class of serum proteins that can be identified by electrophoresis. The most significant gamma globulins are antibodies. Gamma globulin injections are sometimes given in an attempt to temporarily boost a patient's immunity. Injections are also given to patients who have been exposed to hepatitis A or measles and to patients who lack the ability to produce gamma globulins because of an immune deficiency, such as in X-linked agammaglobulinemia.

LP18363-9 Monoclonal band observed
A monoclonal band (M-spike, monoclonal protein isotype) is often found in patients with monoclonal gammopathies. Following protein electrophoresis, immunofixation is used to identify the immunoglobulin heavy chain and/or light chain. Monoclonal peaks greater than 3 g/dL and IgG or IgA are seen in patients with Multiple Myeloma (MM). A monoclonal peak greater than 3 g/dL and IgM is seen in patients with macroglobulinemia. An alternate methodology for identifying the monoclonal protein isotype is MALDI-TOF, in which known information regarding the unique mass of each isotype is used to determine and quantify the isotype present in the patient sample based on the mass spectra generated from the sample.

LP6336-4 Immunofixation
Plasma or concentrated urine is deposited on a gel. With an electrical current the proteins are sorted in function of their size, after which antigens for each targeted type of immunoglobulin are deposited on the gel. Thus more or less narrow bands appear on the gel, marking the location of the various immunoglobulines.
Immunofixation tends to replace protein electrophoresis because:
• it is faster (results within three hours);
• it is somewhat more sensitive. Immunofixation may reveal an immunoglobulin missed out by protein electrophoresis, especially at low concentrations (less than 1 gram/litre);
• it can be partially automated and can be used in more laboratories;
• it is more easily read and interpreted.
Immunofixation is however only sensitive to immunoglobulins and is more expensive than protein electrophoresis.

Protein testing is performed to evaluate and monitor liver and kidney function and to detect and diagnose early kidney damage and various other diseases. Elevated protein levels may also be due to an infection, medication, vigorous exercise, or emotional or physical stress. Body fluids contain many different proteins that serve diverse functions such as transport of nutrients, removal of toxins, control of metabolic processes, and defense against invaders. Testing for protein levels may be done as part of a routine physical, a pregnancy workup, in cased of a suspected UTI, as part of a hospital admission, or to evaluate kidney function.
