This test is a measurement of sperm quality based on various sperm parameters including the concentration, morphology, and acrosomal status of motile spermatozoa. The term is based on, but is not limited in use to, the Medical Electronic Systems, SQA-V Gold analyzer.

The sperm motility index (SMI) is determined by an automated, method that reflects the concentration of motile sperm and the intensity of their motility. The SMI shows a good correlation with manual assessment of sperm density, motility, and morphology. PMID: 9466001 SMI > 160 is considered "Good", 80-160 is "Medium", and <80 is "Poor".
