Aminoacyl-tRNA synthetases (ARS) are enzymes that link amino acids to tRNA molecules and therefore play a key role in protein synthesis. Each ARS is specific for a single amino acid. [NCBI Books: NBK22356] Autoantibodies to ARS are a hallmark of connective tissue diseases including anti-synthetase syndrome (ASS) and idiopathic inflammatory myopathies such as polymyositis (PM) and dermatomyositis (DM). ASS, PM and DM have overlapping features, including myositis, arthralgia, skin manifestations and interstitial lung disease. Antibodies to several specific ARS enzymes have been identified, including anti-histidyl (anti-Jo-1), anti-threonyl (anti-PL-7), anti-alanyl (anti-PL-12), anti-glycyl (anti-Ej), anti-isoleucyl (anti-Oj), anti-asparaginyl (anti-Ks), anti-phenylalanyl (anti-Zo), and anti-tyrosyl (anti-Ha). Different antibodies or patterns of antibodies have been associated with specific clinical features. Anti-Jo-1, anti-Ej, and anti-PL-7 antibodies are associated with myositis, including classic DM, clinically amyopathic DM (CADM) and PM. 3-Hydroxy-3-Methylglutaryl-coenzyme A reductase (HMG-CoA reductase or HMGCR) is an enzyme in the cholesterol-synthesis pathway and is the target of HMG-CoA reductase inhibitors, also known as statins. Autoantibodies to HMGCR may be a marker of necrotizing autoimmune myopathy (NAM), which is characterized by muscle fiber necrosis with little or no inflammation and responsiveness to immunosuppressive treatment. Anti-HMGCR positive cases of NAM usually have limited extramuscular involvement, slow onset of muscle disease, and can occur in both adults and children. Due to the insidious onset, pediatric cases of NAM may be initially confused with muscular dystrophy. The anti-HMGCR antibody titer may correlate with muscle strength and creatine kinase level. Some cases of NAM have been diagnosed in patients previously exposed to statins, and in some of these cases, stopping the statin drug was the only intervention necessary for the resolution of disease. However, exposure to statins is not required to diagnose cases of NAM, and first-line treatment typically includes corticosteroids or other immunosuppressive drugs such as methotrexate. Aminoacyl-tRNA synthetases (ARS) are enzymes that link amino acids to tRNA molecules and therefore play a key role in protein synthesis. Each ARS is specific for a single amino acid. [NCBI Books: NBK22356] Autoantibodies to ARS are a hallmark of connective tissue diseases including anti-synthetase syndrome (ASS) and idiopathic inflammatory myopathies such as polymyositis (PM) and dermatomyositis (DM). ASS, PM and DM have overlapping features, including myositis, arthralgia, skin manifestations and interstitial lung disease. Antibodies to several specific ARS enzymes have been identified, including anti-histidyl (anti-Jo-1), anti-threonyl (anti-PL-7), anti-alanyl (anti-PL-12), anti-glycyl (anti-Ej), anti-isoleucyl (anti-Oj), anti-asparaginyl (anti-Ks), anti-phenylalanyl (anti-Zo), and anti-tyrosyl (anti-Ha). Different antibodies or patterns of antibodies have been associated with specific clinical features. Anti-Jo-1, anti-Ej, and anti-PL-7 antibodies are associated with myositis, including classic DM, clinically amyopathic DM (CADM) and PM. MDA5, more formally known as interferon-induced helicase C domain-containing protein 1, is an immune receptor for viral nucleic acids and plays a major role in activating antiviral responses, including the induction of type I interferons and proinflammatory cytokines. Anti-MDA5 autoantibodies, also known as anti-CADM-140, are associated with autoimmune disorders including dermatomyositis and interstitial lung disease. [UniProt: Q9BYX4] Anti-MDA5 autoantibodies are also found in a subset of patients with juvenile dermatomyositis (JDM) who have polyarthritis and ulcerative skin disease but milder muscle disease.
