Coccidioidomycosis, also known as Valley Fever, is caused by two species of soil-dwelling fungi, Coccidioides immitis and C. posadasii. It is common in the southwestern United States, Mexico, and South America and is transmitted by inhalation, frequently after contaminated soil is disrupted, such as by construction or natural events such as earthquakes or dust storms. Infections range from asymptomatic to life-threatening, and the clinical presentation is the same for both species. In about 40% of patients, symptoms begin one to three weeks after exposure and include cough, shortness of breath, headache, fatigue, and rash. Up to 10% of patients develop pulmonary complications or chronic disease, and about one percent of patients develop disseminated infection involving meninges (i.e., meningitis), soft tissues, and bones or joints. Histoplasmosis capsulatum is a dimorphic fungal pathogen with filamentous or yeast forms,depending on nutritional factors and temperature.The M antigen is a well-known glycoprotein used as a diagnostic antigen for acute histoplasmosis as it induces the first precipitins during disease. Monoclonal antibodies (mAbs) to the M antigen have been found to modify the pathogenesis of histoplasmosis.Given the shortcomings of current therapies, the administration of mAbs is being investigated. PMID: 22347215 The H band is less commonly seen than the M band and is indicative of chronic or severe acute infections with H capsulatum.
H. capsulatum is found in nature primarily as a saprophytic mold. The mold form is comprised of hyaline septate hyphae, which produce two different asexual reproduction structures, macroconidia and microconidia. The micronidia are believed to be responsible for disease propagation since their 2-6 µm size is well suited for disposition into the distal alveoli. Infection is typically acquired through inhalation of fungal propagules upon disturbances of contaminated soil or excreta. Environmental and genetic factors influence disease manifestations which can range from asymptomatic or mild influenza-like disease to a disseminated sepsis form. Manifestations depend on the magnitude of exposure, the host's immunological status and the virulence of the infective strain. Reactivation of latent infection can result in significant disease, particularly for immunosuppressed individuals. Despite the availability of potent antifungal drugs, nearly one in ten patients hospitalized with histoplasmosis dies. PMID: 17223625

Pneumocystis jiroveci is a fungus. This parasitic organism was earlier classified as a protozoan, and still is sometimes called by its former name Pneumocystis carinii. It causes the disease pneumocystis pneumonia (PCP), which afflicts people with weakened immune systems, such as children, the elderly, and especially AIDS patients, in whom it is most commonly observed today. It was first recognized in institutional epidemics among malnourished children and infants. The actual life cycle is unknown. Debate continues as to whether or not the parasite is transmitted through latent infection or active infection. Historically it was thought to be latent, with a duration of up to a year, but recent studies suggest an active state. Symptoms of infection include shortness of breath, nonproductive cough, and low grade fever. Death by asphyxia is a consequent threat, as in any pneumonia.
