Matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) is a type of mass spectrometry (MS). MALDI-TOF MS consists of three primary steps: 1) mixing the sample with an organic matrix, which provides a scaffolding and also protons for the ionization process; 2) applying a laser to the matrix, which absorbs energy from the laser and releases it into the sample, which causes the sample to desorb and form singly charged ions; and 3) using a flight tube to analyze the mass of the ions, ultimately resulting in a mass spectrum in which the mass-to-charge is plotted against signal intensity. MALDI-TOF is a soft ionization technique, meaning that molecules remain relatively intact during ionization. Because of this property, MALDI-TOF can be used for protein analysis; in contrast, older MS techniques cause extensive protein fragmentation and therefore cannot be used for this purpose. One application of MALDI-TOF is in clinical microbiology; compared to traditional methods, MALDI-TOF is faster and more accurate in identifying specific microorganisms. PMID: 23824373

An alternate methodology for identifying the monoclonal protein isotype is MALDI-TOF, in which known information regarding the unique mass of each isotype is used to determine and quantify the isotype present in the patient sample based on the mass spectra generated from the sample.

A non-glycosylated protein that is synthesized in the liver parenchymal cells, regulated by colloid osmotic pressure from interstitial fluid surrounding hepatocytes, and catabolized in nearly all organs. Albumin comprises >50% of plasma proteins in plasma, CSF and urine, and therefore is a key contributor to intravascular oncotic pressure and regulating fluid concentration in tissues. Albumin also binds and transports a variety of endogenous substances and medications. Serum albumin measurement is a reflection of hydration and nutritional status, blood oncotic pressure, liver and kidney function, and other chronic diseases, and the ratio of albumin in the CSF and serum can be used to evaluate blood brain barrier disorders. [Tietz] Urine albumin of less than 30 mg/day is considered normal. Microalbuminuria is defined as urine albumin of 30-300 mg/day and is an early sign of kidney damage from disorders such as diabetes or hypertension. Albuminuria is urine albumin >300 mg/day and is associated with overt kidney disease. PMID: 25512779

Gamma globulins, or Ig's, are a class of serum proteins that can be identified by electrophoresis. The most significant gamma globulins are antibodies. Gamma globulin injections are sometimes given in an attempt to temporarily boost a patient's immunity. Injections are also given to patients who have been exposed to hepatitis A or measles and to patients who lack the ability to produce gamma globulins because of an immune deficiency, such as in X-linked agammaglobulinemia.

Beta globulins are a group of globular proteins in plasma that are more mobile in alkaline or electrically charged solutions than gamma globulins, but less mobile than alpha globulins. A non-glycosylated protein that is synthesized in the liver parenchymal cells, regulated by colloid osmotic pressure from interstitial fluid surrounding hepatocytes, and catabolized in nearly all organs. Albumin comprises >50% of plasma proteins in plasma, CSF and urine, and therefore is a key contributor to intravascular oncotic pressure and regulating fluid concentration in tissues. Albumin also binds and transports a variety of endogenous substances and medications. Serum albumin measurement is a reflection of hydration and nutritional status, blood oncotic pressure, liver and kidney function, and other chronic diseases, and the ratio of albumin in the CSF and serum can be used to evaluate blood brain barrier disorders. [Tietz] Urine albumin of less than 30 mg/day is considered normal. Microalbuminuria is defined as urine albumin of 30-300 mg/day and is an early sign of kidney damage from disorders such as diabetes or hypertension. Albuminuria is urine albumin >300 mg/day and is associated with overt kidney disease. PMID: 25512779

Protein testing is performed to evaluate and monitor liver and kidney function and to detect and diagnose early kidney damage and various other diseases. Elevated protein levels may also be due to an infection, medication, vigorous exercise, or emotional or physical stress. Body fluids contain many different proteins that serve diverse functions such as transport of nutrients, removal of toxins, control of metabolic processes, and defense against invaders. Testing for protein levels may be done as part of a routine physical, a pregnancy workup, in cased of a suspected UTI, as part of a hospital admission, or to evaluate kidney function.

Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.
