CA 125 is an antigenic determinant associated with a mucin-like glycoprotein, molecular weight exceeding 500,000 Daltons. This determinant is recognized by the murine monoclonal antibody OC 125, which reacts with over 80% of epithelial ovarian cancers. Elevated CA 125 levels have been reported in subjects with serous, endometrioid, clear cell, and undifferentiated ovarian carcinoma adenocarcinoma of the endometrium or fallopian tube, as well as some nongynocologic malignancies and certain nonmalignant conditions. Elevated levels also have been observed in pregnant women, most often during the first trimester. CA 125 levels may have significant value in monitoring ovarian carcinoma patients. A persistently rising CA 125 value may be associated with progressive malignant disease and poor therapeutic response. A declining CA 125 value is indicative of favorable prognosis and good response to treatment. CA 125 levels may help predict whether an invasive second-look procedure such as laparoscopy or laparotomy is required after treatment for epithelial ovarian cancer.

Human epididymis protein 4 (known as HE4, or as WAP four-disulfide core domain protein 2) is a protein that in humans is encoded by the WFDC2 gene and belongs to the family of whey acidic four-disulfide core (WFDC) proteins. HE4 is overexpressed in both ovarian and endometrial cancers and is an important biomarker that complements CA 125 in discriminating between benign and malignant pelvic masses, monitoring response to treatment, and detecting recurrences of both ovarian and endometrial cancer. The risk of ovarian malignancy algorithm (ROMA) includes CA 125, HE4, and menopausal status to distinguish benign from malignant adnexal masses. HE4 may augment the sensitivity of CA 125 for detecting early-stage ovarian cancer. PMID: 23909069

The Risk of Ovarian Malignancy Algorithm (ROMA) is a qualitative serum test that combines the results of human epididymis protein 4 (HE4) by EIA, testing for cancer Ag 125 and the patient's menopausal status into a numerical score. The ROMA score is intended to aid in assessing whether a premenopausal or postmenopausal woman who presents with an ovarian adnexal mass is at a low or high likelihood of finding malignancy at surgery. ROMA is indicated for women who meet the following criteria: over age 18; ovarian adnexal mass present for which surgery is planned, and not yet referred to an oncologist. The ROMA score must be interpreted in conjunction with an independent clinical and radiological assessment.
