Term Description
The Patient Examination portion of the General Practitioner assessment of Cognition (GPCOG) is a cognitive assessment for recall, time orientation, clock drawing, and ability to provide current news information. PMID: 11943052 The screening tool is designed for primary care settings. Scores between 0-9 where 9 indicates no significant cognitive impairment (all answers are correct), 5-8 is equivocal, and 0-4 indicates cognitive impairment. If the patient scores 5-8, the patient's informant completes the informant questionnaire [LOINC: 79116-0]. If the patient scores 0-3 out of 6 on the informant questionnaire, cognitive impairment is indicated.

The total score for the GPCOG Patient Examination is equal to the total number of correct answers out of 9 questions. If patient scores 9, no significant cognitive impairment and further testing not necessary. If patient scores 5-8, more information required. Proceed with Step 2, informant section. Term Description
The Informant Interview section of the General Practitioner Assessment of Cognition (GPCOG) screening test [LOINC: 79115-2] is performed when the patient scores between 5-8 out of 9 on the patient GPCOG cognitive assessment [LOINC: 79117-8 ]. To determine if the patient has cognitive impairment, the patient's informant is asked six questions about how the patient is compared to when s/he was well (e.g. 5 - 10 years ago). The total score is equal to the number of items answered 'no', 'don't know' or 'N/A'. If the patient scores between 0-3 out of 6, cognitive impairment is indicated.
