Term Description

Bacterial identification by MALDI-TOF (matrix-assisted laser desorption ionization time of flight) is a diagnostic test for identifying the specific organism(s) responsible for a bacterial infection. MALDI-TOF is a form of mass spectrometry that analyzes the proteins contained in an isolated bacterial colony. The resulting mass spectrum is compared to a reference database of bacterial protein mass spectra in order to identify the organism(s) present in the test sample. This term was created for, but is not limited in use to, the Bruker Daltonics MALDI Biotyper CA System, which is indicated for use in diagnosing Gram-negative bacterial infections.

They are unicellular prokaryotic microorganisms which generally possess rigid cell walls and multiply by cell division. They exhibit three principal forms: round or coccal, rodlike or bacillary, and spiral or spirochetal. Bacteria can be classified by their response to oxygen (aerobic, anaerobic, or facultatively anaerobic) and by how they obtain their energy (chemotrophic, i.e. via a chemical reaction or phototrophic, via a light reaction). In addition, chemotrophs obtain their energy from chemicals, lithotrophic from inorganic compounds and organotrophic from organic compounds. Matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) is a type of mass spectrometry (MS). MALDI-TOF MS consists of three primary steps: 1) mixing the sample with an organic matrix, which provides a scaffolding and also protons for the ionization process; 2) applying a laser to the matrix, which absorbs energy from the laser and releases it into the sample, which causes the sample to desorb and form singly charged ions; and 3) using a flight tube to analyze the mass of the ions, ultimately resulting in a mass spectrum in which the mass-to-charge is plotted against signal intensity. MALDI-TOF is a soft ionization technique, meaning that molecules remain relatively intact during ionization. Because of this property, MALDI-TOF can be used for protein analysis; in contrast, older MS techniques cause extensive protein fragmentation and therefore cannot be used for this purpose. One application of MALDI-TOF is in clinical microbiology; compared to traditional methods, MALDI-TOF is faster and more accurate in identifying specific microorganisms. The mecC gene, a homologue of mecA, is found in bacterial cells and enable bacteria to be resistant to antibiotics such as oxacillin. The gene is carried on a mobile genetic element called the staphylococcal cassette chromosome mec (SCCmec), which integrates at an open reading frame (orfX) in the bacterial genome. Detection of the gene in clinical specimens suggests the presence of methicillin-resistant bacteria, primarily Staphylococcus aureaus (MRSA). Detection of Staphylococcus aureus (SA) and methicillin-resistant Staphylococcus aureus (MRSA) DNA in isolates or clinical specimens by molecular methods. This term was created for, but not limited in use to, the Cephid Xpert MRSA/SA Skin and Soft Tissue Infection Assay (Xpert MRSA/SA SSTI Assay), which is indicated for use in conjunction with other laboratory tests to aid in the detection of MRSA/SA.
