Chloride is an essential mineral required by humans that is found mainly in body fluids. It is an important, negatively charged ion in the blood, and a major electrolyte that helps to conduct electrical impulses. Electrolytes including chloride are located in the blood, lymph, and intra- and extra-cellular fluid. Chloride helps to regulate osmolarity by offsetting the positively charged sodium and potassium ions in the serum. It combines with hydrogen to form hydrochloric acid in the digestive tract which breaks down proteins, absorbs metallic minerals, and activates intrinsic factor. Chloride is absorbed both actively and passively in order to help maintain extracellular fluid volume. Sodium (Na) is a positively charged electrolyte and a mineral. Eighty-five percent of Na is located in the blood and lymph fluid. The hormone aldosterone that is secreted by the adrenal glands helps to regulate Na levels by signaling the kidneys to either retain or secrete it. Damage or disease to the kidneys can lead to dangerous fluctuations of sodium. Na plays a number of key roles in the homeostasis of the human body. It helps to maintain blood volume by osmotically pulling water into the blood vessels. Too much sodium in the blood may cause too much water to be brought into the blood vessels and lead to high blood pressure. Muscles and nerves conduct electrical impulses that require charged particles including Na. The electrical currents cause muscle cells to contract and nerve cells to relay electrical signals between them. Too little sodium can lead to muscle spasms, cramps, headache, irritability, restlessness, nausea and fatigue. Extreme hyponatremia can lead to confusion, hallucinations, decreased consciousness and coma. Conversely, hypernatremia can cause lethargy or restlessness, deep tendon reflexes, muscle spasticity and seizures.

Low ionized calcium levels are common in critically ill patients with sepsis, renal failure, cardiac failure, pulmonary failure, post-surgery or burns. Monitoring of ionized calcium is particularly important in the unconscious or anesthetized patient, in whom unrecognized changes in calcium homeostasis may result in serious cardiovascular dysfunction with little of no prior warning signs. Decreased ionized calcium levels between 3 and 4 mg/dL are usually well tolerated, but the risk of cardiac arrest increases when ionized calcium levels approach 2.5 mg/dL. An ionized calcium level below 2.8 mg/dL is a reasonable threshold to begin calcium replacement therapy. Patients with hypotension or low cardiac output may require calcium replacement when ionized calcium falls below 3.2 to 3.6 mg/dL. Ionized calcium levels should be monitored during replacement therapy.

An elevated blood glucose level is called hyperglycemia and a deficient blood glucose level is called hypoglycemia. Term Description
Knowing the concentration of inspired O2 is important to the interpretation of PO2 results. Inspired O2 can be recorded in two ways- as percent- of O2 delivered to the patient (Venturi mask, re-breather mask, or ventilator) or as liters of oxygen per minute (nasal canula). This panel includes terms for recording the inspired O2 in either fashion. But because only one will apply and labs do not always receive this information they are designated as optional.
The term "oxygen saturation" is often used to refer to two distinctly different quantities, hemoglobin oxygen saturation (recommended symbol = sO2) and fractional oxyhemoglobin (recommended symbol = FO2Hb). The former is sometimes called functional oxygen saturation and the later, fractional oxygen saturation
Hemoglobin oxygen saturation = is the amount of oxyhemoglobin expressed as the percent of the hemoglobin able to bind oxygen, i.e. oxyhemoglobin (O2Hb) + deoxyhemoglobin (HHb).
sO2 = 100 x O2Hb/ (O2Hb + HHb)
Fractional oxyhemoglobin = the amount of oxyhemoglobin expressed as a percent of the total hemoglobin, where tHb = O2Hb + HHb + [COHb + MetHb + SulfHb]. Note that COHb, MetHb, and SulfHb, cannot carry oxygen and are thus called dyshemoglobins.
FO2Hb = O2Hb/tHb
Gas machines that can measure carbon monoxide, could, in principle, also report FO2Hb, and we had assumed that they would. However, at least some machines that do measure COHb and MetHb provide no option for reporting FO2Hb, and report only sO2. When mapping blood gas panels that report COHb% and also report FO2Hb, map that term to the LOINC term with the component of "Oxyhemoglobin /Total hemoglobin". If such an instrument reports only "oxygen saturation, verify with the lab that they are really reporting a functional O2 sat and not the Fractional Oxyhemoglobin, because the two are not always correctly labeled. If it is sO2 then map it to the LOINC term with a component of "Oxygen saturation".
When you are mapping data from a simpler Blood gas instrument (one that can read fewer than 4 wave lengths), then the O2 saturation will likely be a sO2. So map also moat that variable to the LOINC with a component of "Oxygen Saturation".

Be very careful about mapping to this term. We cannot confirm that anyone reports it in this manner - perhaps as number of molecules of oxygen in a given volume. We also cannot confirm that there is a method for measuring oxygen content directly (rather than as the calculation given under the term 59274-1). Likewise we cannot prove that such an approach does not exist. However, all of the usages that we know about are based on calculations. So be very careful about mapping to oxygen saturation terms that are not derived from calculations, e.g. this term and LOINCs 34163-6 and 19219-5.

Oxygen content is a component of a blood gas analysis test that helps to evaluate acid-base balance, gas exchange, and voluntary respiratory control. In the blood gas analysis test, blood is drawn from an artery and analyzed in the lab for pH, partial pressure of carbon dioxide, partial pressure of oxygen, bicarbonate content per 100 mL of blood, oxygen saturation, and oxygen content. Oxygen content is the sum of the oxygen that is combined with hemoglobin and the oxygen dissolved in the plasma. Bicarbonate is an electrolyte and typically measured along with other electrolytes (e.g. sodium, potassium, and chloride) to detect, evaluate, and monitor electrolyte imbalances. Low bicarbonate levels may occur in patients with Addison's disease, chronic diarrhea, diabetic ketoacidosis, and kidney disease. Increased levels may be seen in patients with severe vomiting, cushing syndrome, and metabolic alkalosis. Various drugs may also increase or decrease biocarbonate levels.[Lab Tests Online: co2]
Bicarbonate and total CO2 are not synonyms. Total CO2 = bicarbonate + CO2 + carbonic acid. Under normal circumstances, bicarbonate is the primary constituent of total CO2, however, in certain pathologic conditions, CO2 and/or carbonic acid contribute a clinically significant fraction of the total.

CO2 content is a measurement of total CO2 in the blood. Most of it is in the form of bicarbonate (HCO3) controlled by the kidney. A small amount (5%) of CO2 is dissolved in the blood as insoluble carbonic acid (H2CO3). Changes in CO2 content generally relate to metabolic issues, renal function and unusual losses (diarrhea). Base excess refers to the amount of a strong acid required to bring back the blood pH to the normal value of 7.4. The term and concept were first introduced by Astrup and Siggaard-Andersen in 1958 as a way to measure the metabolic component of an acid-base disturbance.
Actual versus standard:
In vivo, acid-base homeostasis takes place throughout the extracellular fluid (ECF), not just the blood compartment that is in a patient specimen. The concept of "standard" base excess attempts to adjust for the base excess of the entire extracellular compartment by calculating the base excess for blood diluted as if the entire extracellular compartment were sampled (approx Hgb of 5g/l). So the ACTUAL base excess (ABE) is the dose of acid to return PLASMA to normal pH, whereas the STANDARD base excess (SBE) is the dose to return ECF to normal pH.
Adjusting for oxygen saturation:
Base excess normally assumes 100% oxygen saturation, which is valid for many arterial specimens. However, it is possible to add additional terms to the calculation which account for the actual oxygen saturation of the patient sample. The terms in LOINC indicating 'Base excess.100% oxygenated' are intended to encode base excess terms adjusted for oxygen saturation (i.e. do not assume 100% oxygenation).

In human physiology, base deficit refers to a decrease in the total serum concentration of bicarbonate. It can be indicative of metabolic acidosis (acidosis describes the abnormal conditions that results from an excess of acid within the blood such as in a case of lactic acidosis due to anaerobic metabolism) or compensatory respiratory alkalosis (alkalosis describes a conditions that results from an excess alkali (base) within the blood). A base deficit indicates an excess of acid. It refers to the amount of base needed to titrate a serum pH back to normal (healthy human-arterial blood pH varies between 7.35 and 7.45) when the contribution of respiratory factors is taken out of the equation.
Base deficit is usually reported as a negative base excess. Therefore a base excess of -5 is a base deficit of 5.

Oxygen saturation (sO2) is defined as the ratio of oxyhemoglobin to "active" hemoglobin (oxyhemoglobin + deoxyhemoglobin). Some say the term oxygen saturation has been erroneously redefined with a denominator of "total" instead of "active" hemoglobin, an ambiguity that can sometimes create confusion when viewing reported values from different instruments (Anesth Analg. 2007 Dec;105(6 Suppl):S5-9.) Oxygen saturation is commonly measured by blood gas analyzers and pulse oximetry.
