Bordetella pertussis is a species of Bordetella that causes pertussis, also known as whooping cough. Pertussis causes coughing paroxysms, whooping, and post-tussive emesis. Vaccination against B. pertussis can prevent disease; however, B. parapertussis, another species of Bordetella, also causes illness that is often indistinguishable from pertussis, and the B. pertussis vaccine does not protect against B. parapertussis infection. Bordetella parapertussis is a species of Bordetella and is increasingly being identified as the cause of pertussis-like symptoms, including coughing paroxysms, whooping, and post-tussive emesis. Haemophilus influenza is a pleomorphic gram-negative coccobacillus. H influenzae is divided into six serotype (A-F) based on their capsular polysaccharides. The majority of haemophilus infections are caused by H influenza serotype B (HIB) which causes pneumonia, bacteremia, meningitis, epiglottitis, septic arthritis, cellulitis, otitis media, purulent pericarditis, endocarditis and osteomyelitis. Infections with the remaining serotypes lead to similar disease processes as HIB. Unencapsulated H influenzae that is not able to be typed (nontypable H influenzae) can cause ear infections in children and bronchitis in adults but also bacteremia and bacteremic pneumonia. Nontypable H influenza is the main cause of invasive H influenzae infections in all age groups. Transmission of H influenzae is by direct contact with respiratory aerosols. Legionella pneumophila, a gram-negative bacillus, most commonly causes acute pneumonia (legionellosis). Transmission to humans is usually through aerosols of infected water sources such as fountains, misting systems, and drinking water. Severity of infections due to L. pneumophila range from mild (walking pneumonia) to fatal (multilobar pneumonia). Antimicrobial therapy usually includes erythromycin, with the addition of rifampin in severe cases. Initial response to therapy is usually rapid, but full recovery can take weeks to months. Moraxella (Branhamella) catarrhalis, previously known as Neisseria catarrhalis or Micrococcus catarrhalis, is a gram-negative, aerobic diplococcus frequently found in the respiratory tract. M. catarrhalis may cause a variety of illnesses, ranging from sinusitis, otitis media, lower respiratory tract infections, in adultsconjunctivitis and keratitis in children, to laryngitis, bronchitis, pneumonia and lower respiratory tract infections, particularly in those with chronic obstructive pulmonary disease (COPD). A combination of serological and conventional microbiological studies is used to confirm infection by M. catarrhalis. Up to 75% of children carry M. catarrhalis, while carriage rates in adults is extremely low (1-3%). PMID: 11781271

Mycoplasma pneumoniae is a filamentous cell without a cell well. It has an attachment organelle at its tip with protein adhesions which attach to sialoglycoproteins or sialoglycolipid receptors on the tracheal epithelium. Pneumonia is induced by local immunologic and phagocytic responses.
Mycoplasmas generally are the smallest self-replicating organisms possessing the smallest genomes (500 to 1000 genes). Mycoplasmas have several surface antigens; antibodies to these surface antigens inhibit their growth. While various serological tests have been developed and are useful in classification, the infections are persistent, frequently difficult to detect and difficult to cure. Mycoplasma pneumonaie infection affects the upper and lower respiratory tracts. Symptoms including cough, fever and headache may persist for several weeks. Streptococcus pneumoniae are gram-positive cocci that are nonmotile and nonsporulating and usually require complex culture media. Older streptococcal cultures, beyond the most active growth period (the logarithmic phase), sometimes lose their gram-positive staining characteristics. S. pneumoniae are part of the normal respiratory tract flora. Disease usually results due to the acquisition of a new strain following a change in the normal flora, often due to a preceding virus. S. pneumonaie causes otitis media, sinusitis and pneumonia in the middle ear, sinuses or lungs, respectively. Meningitis can occur by bloodstream invasion from the lungs or by extension of otitis media or sinusitis to the meninges. Bacteremia can result in osteomyelitis (infection of bones) or arthritis (of joints). Most bacterial pneumonia occurring after the neonatal period is caused by S. pneumoniae. Pneumococcal pneumonia is seen most commonly in elderly or immunosuppressed individuals. In the United States, it is most prevalent in winter. Diagnosis is usually based on a sputum Gram stain and culture although blood or cerebrospinal fluid may be cultured. While penicillin has been the standard treatment, some strains have proven resistant to penicillin and multiple antibiotics. A vaccine exists. [NCBI Books:NBK7611]

Adenoviruses, medium-sized (90-100 nm), non-enveloped icosohedral viruses with double-stranded DNA, have over 50 immunologically distinct types that cause infections in humans. Adenoviruses usually cause respiratory illness with symptoms ranging from the common cold to pneumonia, croup, and bronchitis. Adenoviruses can cause other illnesses such as gastroenteritis, conjunctivitis, cystitis, and less commonly, neurological disease. Infants and individuals with weakened immune systems are at high risk for severe complications of adenovirus infection and adenoviruses commonly cause acute respiratory illness in military recruits. Some asymptomatic people infected with adenoviruses have ongoing infections in their tonsils, adenoids, and intestines and can shed the virus for months or years. The virus can persist outside the body for long periods. Adenovirus infections can be detected using antigen detection, polymerase chain reaction assay, virus isolation, and serology. Typing is usually done by hemagglutination-inhibition and/or neutralization with type-specific antisera or by molecular methods. As of October 2014 there is no adenovirus vaccine for the general public, however an oral vaccine against types 4 and 7 was approved by the U.S. Food and Drug Administration in March 2011 for U.S. military personnel. The vaccine is recommended by the U.S. Department of Defense for military recruits entering basic training in order to prevent acute respiratory disease. Most adenovirus infections are mild and typically require only treatment of symptoms.

Human bocavirus (HBoV) is a parvovirus that was first identified in 2005 and has been found in a variety of specimen types, including respiratory, stool, blood, and urine. It primarily causes respiratory symptoms in young children under the age of two. HBoV, like other paroviruses, is a small, icosahedral, non-enveloped DNA virus. Four subtypes of HBoV have been identified (HBoV 1-4), with subtypes 1-2 being the most common cause of illness. Subtype 1 is most often found in respiratory specimens, and types 2-4 in stool specimens. Transmission and infection typically occur in the winter and spring. PMID: 27818586

Human metapneumovirus (hMPV) is a Paramyxoviridae family virus that causes upper and lower respiratory tract infections in the very young, elderly and those with compromised immune systems. hMPV can vary in its size, shape and staining properties, and shares similar structure with avian pneumovirus (aMPV) and human respiratory syncytial virus (hRSV). Typically, molecular testing methods, like reverse transcriptase PCR (RT-PCR), are used to detect hMPV. hMPV is thought to be responsible for about 5% to 10% of pediatric hospitalizations of those suffering from acute respiratory tract infection. Human infections with a new avian influenza A (H7N9) virus were first reported in China in March 2013. Most of these infections are believed to result from exposure to infected poultry or contaminated environments, as H7N9 viruses have also been found in poultry in China. While some mild illnesses in human H7N9 cases have been seen, most patients have had severe respiratory illness, with about one-third resulting in death. Rare, limited person-to-person spread of this virus has been identified in China, but there is no evidence of sustained person-to-person spread of H7N9. The first case outside of China was in Malaysia and was reported on February 12, 2014. The case was detected in a traveler from an H7N9-affected area of China. The new H7N9 virus has not been detected in people or birds in the United States as of this writing.

Influenza viruses, from the family Orthomyxoviridae, are enveloped negative-strand RNA viruses with segmented genomes containing seven to eight gene segments. Of the five genera in the family, only influenza A virus (IAV), influenza B and C are medically relevant to humans. Aquatic birds harboring avian IAV are the likely natural reservoir for all known IAV subtypes and are probably the original source of all human pandemic IAV strains. IAV maintained in wild birds have been associated with stable host switch events to novel hosts, including most often to domestic poultry but also to horses, swine, toothed whales and humans. The influenza virus life cycle begins with the binding of viral HA to host cell receptors. Endocytosis and HA-mediated fusion of the viral and cellular membranes occurs. Viral ribonucleoprotein complexes are released into the cytoplasm, travel to the nucleus, then are replicated and transcribed by the viral polymerase complex. The new viruses are formed and bud when the newly formed ribonucleicprotein complexes and structural proteins reach the plasma membrane. Further research is needed to better understand how IAVs switch hosts and the viral genetic changes that underlie human adaptation. IAV causes respiratory infections in humans ranging from asymptomatic to fatal. Influenza viruses A and B are single-stranded RNA enveloped viruses belonging to the Orthomyxoviridae family. The symptoms of infection of Influenza A and B include fever, cough, sore throat, rhinorrhea, nasal congestion, headache, myalgia and malaise. Some patients may experience gastrointestinal symptoms including watery diarrhea, vomiting and abdominal pain. The influenza viruses spread rapidly in enclosed spaces with a documented rate of infection of 70% following a common source of exposure. Infection occurs in the respiratory epithelium in both the upper and lower respiratory tract, and the virus can be found in many specimen types, including nasal aspirates, nasal or nasopharyngeal swabs, throat swabs, sputum and tracheal aspirates. Influenza virus from the family Orthomyxoviridae are enveloped negative-strand RNA viruses with segmented genomes containing seven to eight gene segments. Of the five genera in the family, only influenza A virus (IAV), influenza B and C viruses are medically relevant to humans. Influenza A and B viruses have a similar structure whereby these viruses each contain eight discrete single-stranded RNA gene segments, each encoding at least one protein. While having a close phylogenetic relationship, influenza virus types A and B manifest very differently. While IAV infects birds, humans and other mammals, influenza B only infects humans and seals. Although both viruses cause human respiratory infection seasonally in temperate climate regions and more continually in the tropics, influenza B is normally less prevalent and causes less severe disease than IAV viruses. Middle East Respiratory Syndrome (MERS) is an illness caused by the virus Middle East Respiratory Syndrome Coronovirus (MERS-CoV). The illness was first reported in Saudi Arabia in 2012, but was later found to have originated in Jordan in 2012. MERS has been found in camels leading to speculation that they may be the initial reservoir for the virus. The largest outbreak of MERS that has occurred outside of the Arabian Peninsula was in the Republic of Korea in 2015 and was linked to an individual who had been traveling in the Arabian Peninsula. The virus is probably spread through respiratory secretions from infected individuals though the exact mode of transmission is under investigation. Infection with MERS begins with an incubation period of 5-6 days followed by a severe acute illness that involves fever, shortness of breath and for patients with comorbidities or reduced immunity can lead to pneumonia and kidney failure. The mortality rate of MERS is 3-4 out of 10 people who are infected, though most of the people who die have underlying conditions. In addition, some people experience only cold-like symptoms and others have no symptoms at all. In the United States, most state laboratories are approved to test for MERS-CoV using the real-time reverse transcription-PCR assay developed by the CDC. The assay can be run on respiratory, serum and stool specimens.[CDC: coronavirus/mers]

Human parainfluenza viruses (HPIVs) are a group of four distinct serotypes of single-stranded RNA viruses belonging to the paramyxovirus family. They are the second most common cause of lower respiratory tract infection in younger children. Repeated infection throughout the life of the host is not uncommon. Symptoms of later breakouts include upper respiratory tract illness as in a cold and sore throat. The incubation period of all four serotypes is 1 to 7 days. Parainfluenza viruses can be detected via cell culture, immunofluorescent microscopy, and PCR. Though no vaccines currently exist, research into vaccines for HPIV-1, -2, and -3 is underway. Parainfluenza viruses last only a few hours in the environment and are inactivated by soap and water.

The respiratory syncytial virus (RSV) is a member of the Paramyxoviridae family and is an enveloped pleomorphic virus. RSV is spread through contact with respiratory droplets, contact with the patient and contact with contaminated surfaces. The rate of transmission is affected by outside temperature, humidity and precipitation. RSV infection spreads faster when the temperature is low and the humidity is high in temperate zones and when rainfall levels are high in tropical climates. RSV causes severe lower respiratory infections in infants and young children. Infants born prematurely are particularly susceptible. Symptoms of RSV infection include cough, low-grade fever and rhinorrhea. Human rhinoviruses (HRVs) can be categorized into three genetically distinct HRV groups, designated groups A, B, and C, within the genus Enterovirus and the family Picornaviridae. HRVs are positive-sense, single-stranded-RNA (ssRNA) viruses of approximately 7,200 bp. HRVs, which are the cause of more than one-half of cold like illnesses, are commonly associated with upper respiratory tract infection, otitis media, and sinusitis. PCR testing for respiratory virus detection has led to the recognition of HRV as a lower respiratory tract pathogen as well, especially in patients with asthma, infants, elderly patients, and immunocompromised individuals. At this time there are no approved antiviral therapies, and treatment is primarily supportive.

The Severe acute respiratory syndrome (SARS) coronavirus 2 (SARS-CoV-2), first known as 2019 Novel Coronavirus (2019-nCoV), was first identified as the cause of a respiratory illness outbreak, later defined as Coronavirus Disease 2019 (COVID-19), in Wuhan, China. Real time Reverse Transcription-Polymerase Chain Reaction (rRT-PCR) tests targeting regions within genes, including RdRp, S, N, M, and E genes, specific to SARS-CoV-2 have been developed by the Centers for Disease Control and Prevention (CDC), the World Health Organization (WHO) and others for the diagnosis of COVID-19. The Severe acute respiratory syndrome (SARS) coronavirus 2 (SARS-CoV-2), first known as 2019 Novel Coronavirus (2019-nCoV), was first identified as the cause of a respiratory illness outbreak, later defined as Coronavirus Disease 2019 (COVID-19), in Wuhan, China. The full genome of the SARS-CoV-2 that causes COVID-19 was first posted by Chinese health authorities in GenBank, the NIH genetic sequence database, and in the Global Initiative on Sharing All Influenza Data (GISAI) portal. This facilitated detection of the virus globally with subsequent sequences being nearly identical, suggesting a single, recent emergence from a virus related to bat coronaviruses and the SARS coronavirus. Real time Reverse Transcription-Polymerase Chain Reaction (rRT-PCR) tests targeting regions within genes, including RdRp, S, N, M, and E genes, specific to SARS-CoV-2 have been developed by the Centers for Disease Control and Prevention (CDC), the World Health Organization (WHO) and others for the diagnosis of COVID-19. The 2019 Novel Coronavirus (2019-nCoV) is a coronavirus first identified as the cause of a respiratory illness outbreak in Wuhan, China. Initially, the outbreak was linked to a large seafood and animal market and thought to only be spread by animal to person contact. Within a short time, a growing number of patients who had not been exposed to animal markets were diagnosed with the infection, indicating person-to-person transmission. As of January 2020, infections with 2019-nCoV had been reported internationally due to confirmed cases in travelers from Wuhan. Overall, clinical symptoms appear milder than SARS Coronavirus (SARS-CoV) and Middle East Respiratory Syndrome (MERS) Coronavirus (MERS-CoV) infections as do the fatality rate and transmissibility. PMID: 31953166 Reported illnesses range from minor or no respiratory symptoms to severe acute respiratory illness and death.
