The disease results from the ingestion food contaminated with enterotoxin-producing C perfringens, usually C perfringens type A. Incubation time is 8-22 hours after ingestion of contaminated food, and symptoms include diarrhea, cramps, and abdominal pain. Escherichia coli (EC) is a gram-negative, flagellated, rod-shaped bacterium. [https://vetmed.iastate.edu/vdpam/FSVD/swine/index-diseases/colibacillosis] The human gastrointestinal (GI) tract is colonized by EC within hours of birth. The natural barriers of the GI tract prevent infection by symbiotic EC except when they break down such as in peritonitis. EC occupy the mucous layer of the mammalian colon and is the most numerous anaerobe of the intestinal microflora. Some strains of EC have evolved virulence factors that precipitate serious disease including enteric diarrhea, urinary tract infections, sepsis and meningitis. The intestinal pathogens of EC include six pathotypes defined by the virulence factors they express. These include: enteropathogenic E. coli (EPEC), enterohaemorrhagic E. coli(EHEC), enterotoxigenic E. coli (ETEC), enteroaggregative E. coli (EAEC), enteroinvasive E. coli (EIEC) and diffusely adherent E. coli (DAEC). Uropathogenic EC (UPEC) is the most common EC infection that occurs outside the GI tract. The EC pathotypes that infects extraintestinal locations have been designated ExPEC. Haemophilus influenza is a pleomorphic gram-negative coccobacillus. H influenzae is divided into six serotype (A-F) based on their capsular polysaccharides. The majority of haemophilus infections are caused by H influenza serotype B (HIB) which causes pneumonia, bacteremia, meningitis, epiglottitis, septic arthritis, cellulitis, otitis media, purulent pericarditis, endocarditis and osteomyelitis. Infections with the remaining serotypes lead to similar disease processes as HIB. Unencapsulated H influenzae that is not able to be typed (nontypable H influenzae) can cause ear infections in children and bronchitis in adults but also bacteremia and bacteremic pneumonia. Nontypable H influenza is the main cause of invasive H influenzae infections in all age groups. Transmission of H influenzae is by direct contact with respiratory aerosols. Kingella kingae is a common etiologic agent of occult bacteremia and joint and bone infections in young children between the ages of 6 and 36 months as well as opportunistic infections in older children and adults that are immunocompromised. Cases of K. kingae are more commonly diagnosed in developed countries, possibly due to the wider availability of molecular assays for organism detection compared to developing nations. K. kingae is a Gram-negative coccobacillus with tapered ends that is usually arranged in pairs or short chains. A variant of the organism grows as pinpoint colonies and is called a small-colony variant (SCV). K. kingae's virulence mechanisms include pili, polysaccharide capsules, and a toxin named RTX. The genes encoding the RTX toxin (rtxA and rtxB) are two of the molecular targets that are used to detect K. kingae. Other targets include the 16S rRNA and cpn60 genes, both of which are less sensitive than the rtx genes. PMID: 25567222

Morganella morganii is a rod-shaped, anaerobic, Gram-negative bacteria in the Enterobacteriacaeae family commonly found in the environment and human gastrointestinal tract. Despite its wide distribution, the species is considered a rare cause of nosocomial infections and is most commonly associated with post-operative wound and urinary tract infections. Certain clinical isolates are resistance to multiple antibiotics by carrying various resistance markers (e.g.blaNDM-1 and qnrD1). Neisseria gonorrhoeae are gram-negative cocci usually seen in pairs with the adjacent sides flattened. Pili, filament-like appendages, extend from the cell surface and have a role in adherence. Gonorrhea is usually acquired by sexual contact. Men and women aged 15-29 have the highest incidence of gonorrhea. Number of sexual partners, sexual practices and preference, condom use and population mobility contribute to disease incidence. The most frequent sites are the cervix, urethra, rectum, pharynx and conjunctiva. The most common symptom in men is discharge that may be scanty and clear or cloudy to copious and purulent, and often dysuria. Asymptomatic men are an important reservoir for transmission. Endocervical infection is the most common form of infection in women. Ocular infections occur most commonly in newborns who are exposed in the birth canal and can lead to corneal scarring or perforation. Gonococcal bacteremia may lead to disseminated infection including pelvic inflammatory disease in as many as 15 percent of women, leading to an increased probability of infertility and ectopic pregnancy. While antibiotics have successfully treated gonorrhea for decades, N. gonorrhoeae has developed antimicrobial resistance to every drug used for treatment. The Centers for Disease Control and Prevention recommends combination therapy using two antimicrobials with different mechanisms of action (e.g., a cephalosporin plus azithromycin) to improve treatment efficacy and potentially slow the spread of resistance to cephalosporins. Salmonella are gram-negative, flagellated, facultative intracellular, anaerobic bacilli with O, H and Vi surface antigens. Once ingested, Salmonella survives the gastric acid barrier and penetrates the epithelial cells of the small and large intestines. The invasion of the epithelial cells stimulates the release of cytokines and a subsequent inflammatory response leading to diarrhea and possible ulceration of the mucosa.[NCBI Books: 8435] Antibodies to Salmonella are the body's defense when the bacteria is outside of its host cell which occurs at the initial infection and when the bacillus moves from cell to cell. There is a 10 minute window of opportunity when the Salmonella bacillus leaves the macrophage to invade another. During that time, antibodies and complement work to eradicate the bacilli, but some of the extracellular bacilli will evade attack and enter another host cell. Studies have shown that vaccines play an important role in the fight against typhoid fever. PMID: 25566248 There are two species of Salmonella: S. enterica and S. bongori. These species can be further subclassified into subspecies and serovars.
Veterinary Medicine:
Salmonella infection is most often caused by the serovars Salmonella choleraesuis, and Salmonella typhimurium and less often by Salmonella derby. Infected pigs can be asymptomatic and become carriers of S. choleraesius. Typically, the bacteria survive in the mesenteric lymph nodes that drain into the intestines, and are not shed in the feces unless the pig is under stress. The carrier state lasts for weeks to months and is self-limiting. Severe infection with S. choleraesius can be serious starting out as septicemia and progressing to pneumonia and enteritis. The infection can spread to various tissues including to the central nervous system leading to meningitis and to the joints where it manifests as arthritis. Staphylococcus aureus (S. aureus) are cocci-shaped gram-positive bacteria that assemble in grape-like clusters and cause a wide variety of infections. Up to 50% of adults carry S. aureus on their skin or in their mucous membranes. It is highly adaptable, resulting in drug resistant strains: methicillin-resistant S. aureus (MRSA), methicillin-susceptbile S. aureus (MSSA), vancomycin-intermediate S. aureus (VISA) and vancomycin-resistant S. aureus (VRSA). Examples of human infections caused by S. aureus: bacteremia, gastroenteritis, infective endocarditis, meningitis, osteomyelitis, pulmonary infections, septic arthritis, skin and soft tissue infections, toxic shock syndrome, and urinary tract infections. Diagnosis of S. aureus infection may vary by infection site, e.g., clinical signs and symptoms for skin infections, blood or tissue cultures for other infection types. People with chronic health conditions, weakened immune systems or IV drug users are at a greater risk, as are patients in intensive care units (ICUs) and those with a history of certain types of surgeries and patients with implanted medical devices. LP14410-2 Streptococcus pneumoniae
Streptococcus pneumoniae are gram-positive cocci that are nonmotile and nonsporulating and usually require complex culture media. Older streptococcal cultures, beyond the most active growth period (the logarithmic phase), sometimes lose their gram-positive staining characteristics. S. pneumoniae are part of the normal respiratory tract flora. Disease usually results due to the acquisition of a new strain following a change in the normal flora, often due to a preceding virus. S. pneumonaie causes otitis media, sinusitis and pneumonia in the middle ear, sinuses or lungs, respectively. Meningitis can occur by bloodstream invasion from the lungs or by extension of otitis media or sinusitis to the meninges. Bacteremia can result in osteomyelitis (infection of bones) or arthritis (of joints). Most bacterial pneumonia occurring after the neonatal period is caused by S. pneumoniae. Pneumococcal pneumonia is seen most commonly in elderly or immunosuppressed individuals. In the United States, it is most prevalent in winter. Diagnosis is usually based on a sputum Gram stain and culture although blood or cerebrospinal fluid may be cultured. While penicillin has been the standard treatment, some strains have proven resistant to penicillin and multiple antibiotics. A vaccine exists. [NCBI Books:NBK7611]

Streptococcus pyogenes, also known as Group A Streptococcus (GAS), is a species of gram-positive, coccoid, beta-hemolytic bacteria. It is responsible for a wide variety of both invasive and non-invasive diseases including scarlet fever, pharyngitis, impetigo, cellulitis, necrotizing fasciitis and toxic shock syndrome, as well as the sequelae of rheumatic fever and acute poststreptococcal glomerulonephritis. This test is used for the detection of vancomycin resistance genes by multiplex PCR. Vancomycin is a glycopeptide antibiotic used to treat various infections, primarily those caused by gram-positive bacteria. Vancomycin resistance genes were first found in enterococci, and more recently have been found in Staphylococcus aureus. The emergence of vancomycin-resistant enterococci (VRE) and vancomycin-resistant Staphylococcus aureus (VRSA) in hospitalized patients has lead to a restricted use of vancomycin. To date, multiple vancomycin resistance genes have been characterized, including: vanA, vanB, vanC (vanC-1, vanC-2, vanC-3), and VanD. Of the six van types, vanA and vanB are the most resistant to vancomycin, and are the two types that cause VRSA. PMID: 24983424

Non-susceptibility to carbapenems in gram-negative bacilli by means of the enzyme KPC (Klebsiella pneumoniae carbapenemase) is becoming more common. The gene beta-lactamase KPC (bla(KPC)) encodes KPC enzyme production, which in turn hydrolyzes and inactivates carbapenem antibiotics. KPC enzymes are a class A beta lactamases with several variants (KPC-2, KPC-3, etc) known. Various methods are used to detect specific portions of genes encoding KPC production. Carbapenemases can also be detected by other methods, including the modified Hodge test to detect carbapenemase production in isolates and minimum inhibitory concentration (MIC) to determine level of resistance.

New Delhi Metallo-beta-lactamase (NDM) is an enzyme that makes bacteria resistant to a broad range of beta-lactam antibiotics, including those in the carbapenem family. NDM-1 presents as the most dominant type. The NDM gene encodes beta-lactamase enzymes called carbapenemases. The most common bacteria that make the NDM enzyme are Gram-negative such as Escherichia coli and Klebsiella pneumoniae, but the gene for NDM can spread from one strain of bacteria to another by horizontal gene transfer.

Plasmid-mediated IMP-type carbapenemases are currently known to have 17 varieties. Studies have revealed its establishment in Japan in the 1990s in enteric Gram-negative organisms, Pseudomonas species and Acinetobacter species. IMP genes usually co-exist with other resistance genes resulting in multi-drug resistance.
Studies have shown that a high percentage of patients with immunodeficiency and drug resistant nosocomial infections are infected with a VIM or IMP producing organism, particularly when the organism identified is pseudomonas aeruginosa. The two genes are often present together although they can also be present individually. Identifying the presence of these genes is important in monitoring drug resistance and determining the best antibiotic therapy methods.

The existence of OXA-48 like class D beta-lactamases has been increasing in enterobacter species organisms. The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435. The OXA-48 like class D beta-lactamase enzymes are highly resistant to penicillins and beta lactamase inhibitors and show a low resistance to carbapenems. These varied OXA-48-like beta-lactamases have been identified worldwide, including OXA-48-like producing Klebsiella pneumoniae with resistance to multiple drugs that emerged in Jordanian hospitals. Reports have shown that organisms producing OXA-48 like enzymes are either not resistant to broad-spectrum cephalosporins or show a decreased susceptibility to carbapenemase drugs. Identifying them can be a challenge. Screening and detection is required to prevent and control dissemination. PMID: 22499996 PMID: 26993738

The bacterial (CTX-M) gene is targeted to detect CTX-M type beta lactamases, a group of class A extended-spectrum beta-lactamases (ESBLs) that are a common cause of community-acquired infections. They exhibit powerful activity against cefotaxime and ceftriaxoane. CTX-M type ESBLs are rapidly spreading among Enterobacteriaceae worldwide and replacing TEM-type and SHV-type beta lactamases as the predominant ESBLs in many countries. They originate from Kluyvera species and can be separated into 5 different groups: CTX-M-1, CTX-M-2, CTX-M-9, CTX-M-8, and CTX-M-25. CTX-M2, CTX-M3, and CTX-M14 are the most prevelant. More than 80 CTX-M allotypes are currently known (2014).
