Dihydrofolate reductase (DHFR) is an enzyme that changes dihydrofolate to tetrahydrofolate, required for the synthesis of purines, thymidylic acid, and certain amino acids. Due to its critical function, DHFR is found in all organisms. In humans it is encoded by the DHFR gene, and can be the target of drugs such as methotrexate used to treat certain neoplasms and inflammatory diseases. In bacteria, the bacterial DHFR enzyme is targeted by synthetic antibiotics such as trimethoprim. The chromosomal version of the bacterial gene (commonly folA) is sometimes overridden by trimethoprim-resistant versions present on mobile elements. Mutations of DHFR resulted in widespread resistance to trimethoprim. The dfrA class of genes are found to be the largest family of mobile trimethoprim antibiotic resistance genes. This test is used for the detection of vancomycin resistance genes by multiplex PCR. Vancomycin is a glycopeptide antibiotic used to treat various infections, primarily those caused by gram-positive bacteria. Vancomycin resistance genes were first found in enterococci, and more recently have been found in Staphylococcus aureus. The emergence of vancomycin-resistant enterococci (VRE) and vancomycin-resistant Staphylococcus aureus (VRSA) in hospitalized patients has lead to a restricted use of vancomycin. To date, multiple vancomycin resistance genes have been characterized, including: vanA, vanB, vanC (vanC-1, vanC-2, vanC-3), and VanD. The Enterobacter cloacae complex comprises of six phenotypically and genetically heterogeneous species: Enterobacter cloacae, Enterobacter asburiae, Enterobacter hormaechei, Enterobacter kobei, Enterobacter ludwigii and Enterobacter nimipressuralis. Among the Enterobacter spp., the E. cloacae complex species are the most common cause of nosocomial bloodstream infections, with E. cloacae and E. hormaechei most frequently isolated in human clinical specimens. The emergence and spread of antimicrobial resistance in the E. cloacae complex has increased the complexity of treating infections due to these organisms. Enterococcus faecalis and faecium are Gram-positive commensal bacteria of the gastrointestinal tract. Although part of the normal intestinal flora, Enterococcus species are increasingly recognized as important pathogens and a common cause of nosocomial infections. E. faecalis and E. faecium account for the vast majority of infections in humans, which can include urinary tract infections, endocarditis, bacteremia, wounds infections, intraabdominal and pelvic infections. Enterococci can show both intrinsic and acquired resistance to antibiotics including penicillin, chloramphenicol, tetracyclines, rifampin, fluoroquinolones, aminoglycosides and vancomycin. E. faecium is responsible for the majority of vancomycin-resistant enterococcal (VRE) infections. The virulence of the Enterococci is generally considered lower than S. aureus. Escherichia coli (EC) is a gram-negative, flagellated, rod-shaped bacterium. [https://vetmed.iastate.edu/vdpam/FSVD/swine/index-diseases/colibacillosis] The human gastrointestinal (GI) tract is colonized by EC within hours of birth. The natural barriers of the GI tract prevent infection by symbiotic EC except when they break down such as in peritonitis. EC occupy the mucous layer of the mammalian colon and is the most numerous anaerobe of the intestinal microflora. Some strains of EC have evolved virulence factors that precipitate serious disease including enteric diarrhea, urinary tract infections, sepsis and meningitis. The intestinal pathogens of EC include six pathotypes defined by the virulence factors they express. These include: enteropathogenic E. coli (EPEC), enterohaemorrhagic E. coli(EHEC), enterotoxigenic E. coli (ETEC), enteroaggregative E. coli (EAEC), enteroinvasive E. coli (EIEC) and diffusely adherent E. coli (DAEC). Uropathogenic EC (UPEC) is the most common EC infection that occurs outside the GI tract. The EC pathotypes that infects extraintestinal locations have been designated ExPEC. Morganella morganii is a rod-shaped, anaerobic, Gram-negative bacteria in the Enterobacteriacaeae family commonly found in the environment and human gastrointestinal tract. Despite its wide distribution, the species is considered a rare cause of nosocomial infections and is most commonly associated with post-operative wound and urinary tract infections. Serratia marcescens is an opportunistic, gram negative, pathogen from the family Enterobacteriaceae that causes both opportunistic and nosocomial infections. It is estimated that only 1-2% of hospital acquired infections, typically of the respiratory tract, urinary tract, surgical wounds or soft tissues, are due to Serratia marcescens. Serratia-associated meningitis can occur in pediatric patients. Diagnosis of Serratia marcescens infection is made through standard laboratory testing of blood, urine, pus and other bodily fluids. Staphylococcus aureus (S. aureus) are cocci-shaped gram-positive bacteria that assemble in grape-like clusters and cause a wide variety of infections. Up to 50% of adults carry S. aureus on their skin or in their mucous membranes. It is highly adaptable, resulting in drug resistant strains: methicillin-resistant S. aureus (MRSA), methicillin-susceptbile S. aureus (MSSA), vancomycin-intermediate S. aureus (VISA) and vancomycin-resistant S. aureus (VRSA). Examples of human infections caused by S. aureus: bacteremia, gastroenteritis, infective endocarditis, meningitis, osteomyelitis, pulmonary infections, septic arthritis, skin and soft tissue infections, toxic shock syndrome, and urinary tract infections. Diagnosis of S. aureus infection may vary by infection site, e.g., clinical signs and symptoms for skin infections, blood or tissue cultures for other infection types. People with chronic health conditions, weakened immune systems or IV drug users are at a greater risk, as are patients in intensive care units (ICUs) and those with a history of certain types of surgeries and patients with implanted medical devices. Staphylococcus epidermidis, a coagulase negative staphylococcus (CoNS), has been historically viewed as an innocuous commensal organism of human skin. However, it has emerged as an important nosocomial opportunistic pathogen and accounts for up to 90% of all hospital-acquired CoNS infections. S. epidermidis is able to form biofilms on indwelling devices, and isolates are often resistant to a multitude of antibiotics which complicates effective treatment. PMID: 24222452

Staphylococcus saprophyticus (S. saprophyticus) is a gram-positive coccus that is a common cause of uncomplicated urinary tract infections (UTIs). S. saprophyticus is part of the perineum, rectum, urethra, cervix, and gastrointestinal tract human flora. It is also found in pig and cow gastrointestinal flora, which can transfer to humans when consuming respective foods. S. saprophyticus is known to be resistant to Novobiocin. PMID: 29493989

Streptococcus agalactiae (S. agalactiae) or Group B Streptococcus (GBS) is a gram-positive coccus that causes serious diseases, primarily in neonates, pregnant patients, elderly patients and those with significant underlying diseases. S. agalactiae typically presents as skin and soft tissue infections; while meningitis and endocarditis are rare presentations, they are marked with serious morbidity and mortality. Blood cultures confirm S. agalactiae presence. Chronic kidney disease, cardiac failure, history of neoplasia, history of ischemic heart disease, and diabetes mellitus are risks for acquiring agressive S. agalactiae infection.
