The OXA-48 gene encodes one of the three primary classes of beta-lactamase produced by Enterobacteriaceae. OXA-48 is mainly harbored in Klebsiella pneumoniae and occurs mainly in North Africa and Turkey. It has been identified in both nosocomial and community-acquired infections. PMID: 24930781 OXA-48 is a member of the OXA group of class D beta-lactamases which includes six subgroups that are able to hydrolyze carbapenem in different degrees and of which OXA-48 is a member. The Ambler classification for carbapenemases utilizes four molecular classes - A, B, C and D. The OXA group of β-lactamases occur mainly in Acinetobacter species and are part of molecular class D. The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435. Many OXA enzymes (OXA-23-like, OXA-24-like, OXA-48-like, OXA-58-like) are considered to be responsible for the worldwide resistance epidemics. OXA-23-like enzymes have been isolated in carbapenem-resistant Acinetobacter baumannii (CR-AB) in both the US and in China, revealing the importance of national and global efforts to control the spread of CR-AB.

The existence of OXA-48 like class D beta-lactamases has been increasing in enterobacter species organisms. The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435. The OXA-48 like class D beta-lactamase enzymes are highly resistant to penicillins and beta lactamase inhibitors and show a low resistance to carbapenems. These varied OXA-48-like beta-lactamases have been identified worldwide, including OXA-48-like producing Klebsiella pneumoniae with resistance to multiple drugs that emerged in Jordanian hospitals. Reports have shown that organisms producing OXA-48 like enzymes are either not resistant to broad-spectrum cephalosporins or show a decreased susceptibility to carbapenemase drugs. Identifying them can be a challenge. Screening and detection is required to prevent and control dissemination. PMID: 22499996 PMID: 26993738

Oxacillinases, or OXA beta-lactamases, are Class D plasmid-mediated beta-lactamases that hydrolyze oxacillin and related anti-staphylococcal penicillins. Amino acid substitutions in OXA enzymes can also give the extended-spectrum beta-lactamase (ESBL) phenotype. While most ESBLs have been found in E. coli, K. pneumoniae, and other Enterobacteriaceae, the OXA-type ESBLs have been found mainly in P. aeruginosa. The bla(OXA) gene is one of several beta-lactam antibiotic resistance gene markers.

The carbapenemase blaOXA-51-like genes are members of the beta lactam resistance gene group OXA, which causes resistance to carbapenem antibiotics. The blaOXA-51-like genes are primarily found in Acinetobacter baumannii, which is a common cause of multidrug-resistant nosocomial infections. The blaOXA-51- and blaOXA-23-like genes are the two most common resistance genes in A. baumannii and cause resistance to drugs such as ceftazidime, piperacillin, cefepime, nitrofurantoin and ertapenem. PMID: 26550338 The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. Extended-spectrum beta-lactamases (ESBL) are enzymes produced by some gram negative bacilli that confer resistance to the majority of beta-lactam antibiotics, include the penicillins, cephalosporins and the monobactam aztreonam. ESBLs are most often plasmid-borne. Specific ESBLs include TEM, SHV, and the CTX-M groups, which are the most clinically prevalent, and VEB, PER, BEL, and GES, which are less common. PMID: 19246801 -TEM and SHV subtypes are derived from amino acid substitutions, resulting in enzymatic hydrolysis of many beta-lactam antibiotics. -CTX-M genes originate from Kluyvera species. They are separated based on their amino acid sequences into the following groups: CTX-M-1, CTX-M-2, CTX-M-9, CTX-M-8, and CTX-M-25. -VEB, PER, BEL, and GES genes are less common. [Mayo Medical Laboratories] The nomenclature for ESBL genes includes the prefix "bla", which stands for "beta-lactam", followed by the enzyme (ie. blaTEM).

Plasmid-mediated IMP-type carbapenemases are currently known to have 17 varieties. Studies have revealed its establishment in Japan in the 1990s in enteric Gram-negative organisms, Pseudomonas species and Acinetobacter species. IMP genes usually co-exist with other resistance genes resulting in multi-drug resistance.
Studies have shown that a high percentage of patients with immunodeficiency and drug resistant nosocomial infections are infected with a VIM or IMP producing organism, particularly when the organism identified is pseudomonas aeruginosa. The two genes are often present together although they can also be present individually. Identifying the presence of these genes is important in monitoring drug resistance and determining the best antibiotic therapy methods.

Verona integron-encoded metallo-β-lactamase (VIM) is part of a growing family of carbapenemases and includes at least 10 members. VIMs are one of the most common types of carbapenemases, with many variants in at least 23 species of Gram-negative bacilli from more than 40 regions of the world. PMID: 21449842 Studies have shown that a high percentage of patients with immunodeficiency and drug resistant nosocomial infections are infected with a VIM or IMP producing organism, particularly when the organism identified is Pseudomonas aeruginosa. PMID: 27563634 The two genes are often present together although they can also be present individually. Identifying the presence of these genes is important in monitoring drug resistance and determining the best antibiotic therapy methods. PMID: 28085553

New Delhi Metallo-beta-lactamase (NDM) is an enzyme that makes bacteria resistant to a broad range of beta-lactam antibiotics, including those in the carbapenem family. NDM-1 presents as the most dominant type. The NDM gene encodes beta-lactamase enzymes called carbapenemases. The most common bacteria that make the NDM enzyme are Gram-negative such as Escherichia coli and Klebsiella pneumoniae, but the gene for NDM can spread from one strain of bacteria to another by horizontal gene transfer.

Non-susceptibility to carbapenems in gram-negative bacilli by means of the enzyme KPC (Klebsiella pneumoniae carbapenemase) is becoming more common. The gene beta-lactamase KPC (bla(KPC)) encodes KPC enzyme production, which in turn hydrolyzes and inactivates carbapenem antibiotics. KPC enzymes are a class A beta lactamases with several variants (KPC-2, KPC-3, etc) known. Various methods are used to detect specific portions of genes encoding KPC production. Carbapenemases can also be detected by other methods, including the modified Hodge test to detect carbapenemase production in isolates and minimum inhibitory concentration (MIC) to determine level of resistance.

The bacterial (CTX-M) gene is targeted to detect CTX-M type beta lactamases, a group of class A extended-spectrum beta-lactamases (ESBLs) that are a common cause of community-acquired infections. They exhibit powerful activity against cefotaxime and ceftriaxoane. CTX-M type ESBLs are rapidly spreading among Enterobacteriaceae worldwide and replacing TEM-type and SHV-type beta lactamases as the predominant ESBLs in many countries. They originate from Kluyvera species and can be separated into 5 different groups: CTX-M-1, CTX-M-2, CTX-M-9, CTX-M-8, and CTX-M-25. CTX-M2, CTX-M3, and CTX-M14 are the most prevelant. More than 80 CTX-M allotypes are currently known (2014).

LP249101-9 Bacterial colistin resistance mcr-1 gene
The mobilized colistin resistance genes (mcr-1 to mcr-5, and variants) are located on a plasmid within bacterial cells and affect the ability of antibiotics, including polymyxins and colistin, to bind to the bacteria. The mode of action of the antibiotics is to bind to the outer membrane of the phosphate group of lipid A lipopolysaccharide and to intercalate into the inner membrane forming pores that result in bacterial lysis. The polymyxins & colistin are the last line of defense used to treat multidrug resistant gram-negative bacteria so the emergence of the mcr genes in Enterobacteriaceae, including Klebsiella pneumoniae, is onerous and challenging to public health.
